To Identify a Homeless Student

We have a new homeless liaison school assignment list for 2012.2013. We aim to be more streamline with other DPS departments so a regional list was created.  Please view before sending your referral: Homeless Liaison Regional Assignment List 2012.2013  

Our NEW and IMPROVED FORM is the best way to send your student/family referral. ONE form PER STUDENT is required.  EOP staff does our best to respond to requests quickly and will priortize referrals that are sent via 2012-2013 residency questionnaire. Once received by our office we will triage the forms in order of highest need.

School Staff: If siblings attend other DPS schools and the information is not available to complete a form then please note on referral form the siblings and their schools.

We will provide the confidential information to Food Services for free breakfast/lunch eligibilty. Students must participate in the full lunch menu items to receive free lunch. Ala Carte items are not part of free lunch services. 

For Transportation requests please note:  To qualify for transportation, the student must be currently homeless residing outside district walk zones (1 mile for elementary, 2.5 miles middle school, 3.5 miles high school) AND attending their school of origin. Homeless students that participated in DPS SchoolChoice cannot qualify for transportation. Keep in mind that transportation requests are completed in order received and it can take time to get determination by district transportation department. We appreciate your patience as we work on the request.
