Please Complete the Residency Questionnaire--  It includes:

• Student Name
• Student School
• Current address (shelter, motel, doubled up, trans.housing) 
• Resources needed/requesting

We will provide the confidential information to Food Services for free lunch eligibilty and contact the Transportation Department (if requesting and qualifies for transportation). To qualify for transportation student must be residing outside district walk zones.
